Your ACAT assessment
All prospective residents must have their care needs assessed by the Aged Care Assessment Team (ACAT) to determine their eligibility and care needs. To find your nearest ACAT, visit www.myagedcare.gov.au or phone 1800 200 422. You can also visit your GP or local hospital for an ACAT referral.
Members of the ACAT will visit you and ask you about your lifestyle and health to establish the best care option for you. You may like a friend, family member or carer to be present for the assessment. If you choose, your GP will provide the ACAT member with your medical history.
The ACAT assessment is free of charge.
Once you have been assessed by the ACAT, you will receive a copy of your assessment which will advise you on what services you are eligible for. After your assessment, the ACAT member will complete Aged Care Client Record (ACCR) form.
Ensure you complete the “Income Assessment” as soon as possible so you are not incorrectly charged if you choose to enter residential care.
If you are eligible for residential aged care, you will need to provide a copy of your ACAT Assessment when applying to Craigcare.
What comes next?
You can book a tour any of our facilities by contacting the facility manager. You do not need a completed ACAT assessment before booking a tour.
If there is no waiting list for the home and you have completed your ACAT assessment, you may be able secure your place immediately. If the home does have a waiting list, you can add your name to the list. When a suitable room becomes available, one of our friendly staff will contact you.
Your “Resident Agreement” is the most critical document in the admissions process. This document serves as a contract and outlines the rights, responsibilities and financial obligations as one of our residents.
